2025 Changes at a Glance
Helping you and your family reach and maintain good health is very important to Ardent Eagle Solutions. We are pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees. Ardent Eagle Solutions remains committed to providing a competitive, cost-effective benefit program. Highlights of the 2025 benefit plans include:
- All plans are now aligned with the calendar year and will begin on January 1st and remain in effect until December 31st
- NEW! HDHP plan option for medical - Health Savings Account (HSA) eligible plan
- Dental, Vision, Basic and Voluntary Life/AD&D and Disability coverages will remain with Guardian

Open Enrollment
Open Enrollment will take place from November 4th, 2024, through November 15th, 2024. This is the one time of year where you can make changes to your elective coverages with Ardent Eagle Solutions. You may switch between plans, add or remove dependents and drop or add new coverages during the annual open enrollment period. Please also keep in mind the following when making your elections for 2025:
- If you are currently covered by the Voluntary Life/AD&D, you may increase your benefit by $50,000, not to exceed the Guaranteed Issue amount, with no evidence of insurability!
- If you wish to contribute to a Flexible Spending Account (FSA) - Healthcare FSA or Dependent Care FSA - for the 2025 plan year, you MUST log into iSolved and elect your annual contribution amount! Elections for FSAs do not carryover year over year.
- FOR ALL OTHER BENEFITS (Including Parking & Transit Accounts) - THIS IS A PASSIVE ENROLLMENT! If you do not log into iSolved and complete the open enrollment event, current elections will carry forward at the new per paycheck rates, with the exception of the FSA and Dependent Care FSA.

Eligibility & Enrollment
All regular, full-time employees are eligible for benefits through Ardent Eagle Solutions. For benefits purposes only, a regular full-time employee is an employee who is scheduled to work 30 or more hours per week. Lawful spouses, domestic partners, and dependent children may be covered under Ardent Eagle Solutions benefits. For a child to be considered a dependent, he or she must be less than 26 years of age regardless of student status. Stepchildren who reside with you, the employee, and who are primarily dependent upon you for support are also considered eligible dependents. Stepchildren are also subject to the age limitations. A child who has a physical or mental disability may be eligible for coverage at any age with proof of disability.
Coverage is effective on the first of the month following 30 days of continuous employment. Open enrollment takes place each year. This is the time, other than for a qualifying life event, when you can change your benefits elections. During this period, you must determine if you want to make changes to your benefits. If you wish to do so, you must enroll and/or decline coverage(s) for the year. The effective date is January 1st, 2025.
Children are eligible for benefits as follows:
- Medical, Dental and Vision: Until the end of the month in which the child turns 26
- Dependent Child Life Insurance & AD&D: Until the day of their 26th birthday
ENROLLING IN BENEFITS ON iSOLVED:
- Log in to iSolved and go to Employee Self Service, then click on "Benefit Enrollment Wizard".
- Review the company messages and your benefit enrollment end date. Click "Next" to move through the wizard or navigate using the menu on the left. For help throughout the process, use the "Help" button.
- In order to add, modify, or remove contacts, use the "Contact Info" section. First, designate the type of contact. Dependent must be selected to enroll the contact in benefits. Birth date, gender, and SSN are required for dependents. Beneficiary must be selected to assign contacts to life plans.
- Review the "Cost Analysis" for the plans offered by your company. The coverage levels listed will be based on the actual dependents you have entered (for example, if you do not have children, you may not see Family coverage as an option). Click "Print" to keep a copy of this page for your records.
- If applicable, review your current elections. Select "Keep" to continue the same plan/coverage (if available).
- Make your selections in each plan type and use "Next" to continue. A green check will appear when you have completed each type. Use the radio buttons to select a plan or waive coverage.
- After you select a benefit plan, select a coverage level or populate the requested options including assigning dependents and/or beneficiaries. Use "Show All" to view all options for the plan type or "Deselect" to unenroll.
- Use the "Include" checkbox to add dependents and beneficiaries to plans. If your dependent does not appear, go back to the "Beneficiaries and Dependents" screen to add/edit the contact, select the "Dependent and/or Beneficiary" box, then return to the type screen.
- Review your enrollment summary for accuracy. Click "Printable Version" to print. Click "Previous" to make changes. Read the "Action Required information".
- Click "Submit My Benefits" to finalize your selections. A pop-up message will appear asking you to certify that you have reviewed and verified your elections. Click "OK" to submit your elections.
- Submitted elections will be sent to your administrator for final approval. Click "Printable version" to keep the confirmation statement for your records or view in the "Employee Self Service" screen under "Documents".
- You may still make changes until the enrollment period ends by resubmitting your elections.
The Benefit choices you make during your open enrollment remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Events." These are events such as marriage, divorce, birth or adoption of a child, or loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

Medical & Rx

CareFirst Administrators will continue to be your medical and prescription drug carrier. You have the choice between four plans for the 2025 plan year. The four plan options include three PPO plans, with various metallic levels, and a new HDHP plan.
If you are already enrolled in one of the Ardent Eagle medical plans today and have not already done so, please be sure to register through the CareFirst member portal (linked above) where you can access important information pertaining to your medical plans with CareFirst Administrators including participating providers, cost estimates for services, explanation of benefits/claims, and your accumulation amounts for deductibles and out-of-pocket costs. If you are not currently covered, you will be able to register in the CareFirst portal as of your effective date of coverage.
2024 Medical/Rx Updates and Changes:
- PPO Plans: No changes to plan designs - choice of Bronze, Silver or Gold PPO
- New! HDHP Plan: Allows you to contribute to a Health Savings Account (HSA) with pre-tax dollars.

All four medical plans utilize the BlueChoice Advantage network. You will pay the lowest out-of-pocket when you visit physicians and facilities that are in-network. If you do seek care out of network, you will experience higher cost sharing in addition to possible balance billing.

- Navigate to Find Care - Blue Cross Blue Shield Association (bcbs.com)
- Enter your location
- Click Choose a Plan
- Enter the first three letters at the beginning of your member ID number (TTZ)
- Search for a physician/facility by name, specialty, facility, or type
Tax-Advantaged Accounts
You have the opportunity to contribute to a variety of savings and/or spending accounts on a pre-tax basis which lowers your taxable income. View the information below for important details and guidelines for the various accounts offered by Ardent Eagle Solutions.
The Health Savings Account (HSA) allows you to set aside pre-tax dollars to pay for eligible medical, dental, and vision expenses. Upon enrollment in the HDHP medical plan, you become eligible to participate in an HSA. By contributing to an HSA, you reduce your taxable income, meaning you pay less in taxes which in turn saves you money.
The 2025 HSA IRS Maximums are listed below and are based on a calendar year. If you elect the HDHP plan mid-year and want to contribute to an HSA, the maximum contribution limit is prorated based on your effective date in the HSA-eligible plan.
$4,300 for a single contract | $8,550 for a two person or family
$1,000 "Catch Up" contribution for those age 55+
Ardent Eagle Solutions does not have a preferred HSA vendor. You have the freedom to open a Health Savings Account at the bank or financial institution of your choice. Be sure that you open your HSA account as soon as possible following enrollment in the HDHP so that you can begin contributing to the account as soon as possible. Please note, once you've opened your account, be sure to provide your HR team with the account information so your paycheck deductions can be sent directly to your account each payroll.

A Healthcare Flexible Spending Account (FSA) allows you to set aside pre-tax dollars to pay for eligible medical and/or dental and vision expenses. The nice part about an FSA is that you have access to the full annualized amount on day one. This type of FSA is only available if you do not have a Health Savings Account (HSA).
The election you make during open enrollment cannot be changed throughout the year unless you experience a qualifying event that would permit you to make a change. For example, if you get married, you can increase your contribution, however, you cannot decrease or end your FSA for this type of life event.
Plan details are as follows:
- You can contribute up to $3,300 annually. (2025 IRS Maximums TBA)
- At the end of the 2025 plan year, if you have not used all of your FSA funds, a maximum of $660 will carry over into next year's FSA plan (should you elect an FSA for the 2026 plan year).
- You have 90 days after the end of the plan year to submit for expenses that occurred on or before 12/31/2025.
- You have access to your full annualized election amount on day one.
- Funds are use it or lose it! Any funds that you do not use by the end of the 90-day grace period or that exceed the maximum carryover listed above will be forfeited.
- If you leave employment with Ardent Eagle Solutions, you lose the FSA and any funds leftover, except in special circumstances where COBRA continuation of the FSA account may be available.

Flexible Spending Account
The Dependent Care Flexible Spending Account (DCFSA) allows you to set aside pre-tax dollars to pay for eligible dependent care expenses. Qualifying expenses include daycare, nursery & preschool tuition, summer day camps, and more!
The election you make cannot be changed throughout the year unless the change aligns with a qualifying event. For example, you can increase your contribution if you add another member to your family through birth or adoption, however, you cannot decrease your contribution for this type of life event.
Plan details are as follows:
- You can contribute up to $5,000 annually or limited to $2,500 if you are married and file separate tax returns.
- The plan includes a Grace Period which allows you to incur and submit expenses using your 2024 funds until March 15, 2025.
- You have 90 days after the end of the plan year to submit for expenses that occurred on or before 12/31/2025.
- You only have access to the funds of which you have paid into the account.
- Any unused funds are forfeited.
The Parking and Transportation Reimbursement Plan allows you to set aside money on a pre-tax basis to pay for parking and transportation expenses related to your commute to and from work. You can contribute up to $325 a month for parking and up to $325 a month for transportation. You may enroll and change your deferral amounts by the 10th of each month for the next month. You will be able to access this through iSolved.
Dental

Ardent Eagle Solutions' dental plan provides comprehensive coverage to help you and your family maintain good dental health. The dental plan includes both in and out of network benefits. Although your cost sharing is the same in and out of network, if you do go out of network it is likely that you will experience balance billing.
If you are already enrolled in Ardent Eagle Solutions' dental plan today and have not already done so, please be sure to register through the Guardian member portal where you can access important information pertaining to your dental plan with Guardian including participating providers, coverage information, and explanation of benefits/claims. If you are not currently covered, you will be able to register in the Guardian portal as of your effective date of coverage.
You have the choice of one dental plan - the Guardian Dental PPO Plan includes a deductible (for basic and major services only), no ortho coverage, higher cost sharing for your basic and major services, and a lower annual maximum.

What are Preventive Services? Routine oral exams and cleanings, x-rays, sealants & fluoride treatments
What are Basic Services? Periodontics, endodontics (root canals), anesthesia, fillings, periodontal maintenance
What are Major Services? Surgical & simple extractions, crowns, dentures, implants & bridges

Your dental plan also includes Guardian's Oral Health Rewards Program. This program encourages and rewards members who visit the dentist, by rolling over part of your unused annual maximum into a Maximum Rollover Account (MRA). This can be used in future years if your plan's annual maximum is reached. See below for an example of how the maximum rollover works.
Guardian offers a robust dental network: DentalGuard Preferred. If you do choose to visit a nonparticipating dentist you may be balance billed and may have to pay more.

Vision

Ardent Eagle Solutions offers vision coverage through Guardian to help pay for eye exams, prescription glasses and contact lenses. Guardian also provides discounts for Lasik.
If you are already enrolled in Ardent Eagle Solutions Guardian vision plan today and have not already done so, please be sure to register through the Guardian member portal where you can access important information pertaining to your vision plan with Guardian including participating providers, coverage information, and explanation of benefits/claims. If you are not currently covered, you will be able to register in the Guardian portal as of your effective date of coverage.

Your vision network with Guardian is the VSP Choice network. You may use the eye care professional of your choice, however, when you visit a participating in-network provider, you receive higher levels of coverage. If you choose to receive services out-of-network, you may be required to pay that provider at time of service and will need to submit a claim to Guardian for the applicable reimbursement based on the out-of-network reimbursement schedule.

Financial Protection
Ardent Eagle Solutions understands the importance to you and your family to have strong financial protection. We provide all employees Basic Life/AD&D and Disability coverage at no cost to you. In addition to these benefits, we offer a variety of insurance you can choose from on a voluntary, employee paid basis so you can select what is best for your family. All of these coverages are provided to protect your income and out of pocket expenses.

A sudden accident or death can leave you or your loved ones in a vulnerable position. Ardent Eagle Solutions provides all eligible employees Basic Life and Accidental Death & Dismemberment (AD&D) coverage in the amount of $75,000 at no cost to you. Employees have the opportunity to enroll in additional Life and Accidental Death & Dismemberment insurance beyond what Ardent Eagle provides which can supplement lost income and help pay for expenses in the event of an accident or death. If you choose to enroll in Voluntary Life/AD&D coverage, this will be in addition to your employer provided Basic Life coverage.
- Evidence of Insurability (EOI) is required for increases in excess of 1 increment during open enrollment and for those electing voluntary life for the first time after initially waiving the coverage.
- Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
- *Guarantee Issue only applies to new hires or newly eligible spouses (due to marriage) electing benefits for the first time; or, when increasing benefits during open enrollment. You are eligible to increase your benefits by 1 increment, not to exceed the guaranteed issue amount, during the 2024 open enrollment period with no evidence of insurability.
